Client Appointment Information Up

Once you have booked an appointment you will receive our medical questionnaire and instructions in the post.

consult-room-2

What to bring:

Questionnaire (completed please)

Shorts and T-Shirt

Running Shoes

Towel

Blood Tests:

Please ensure you have completed the blood tests at least 1 day prior to your assessment. To achieve an accurate indication of your cholesterol risk you must fast for 12 hours before the blood test.  In other words - no food, coffee, tea, or tobacco from 8:00 pm the evening prior to the test. You should drink a couple of glasses of plain water during this period to prevent dehydration.

Other Tests:

As required we offer other tests as appropriate to your age, sex and risk profile. These may include chest X-ray, bone density, mammography, PSA tests and others.

If we find during your appointment that further evaluation is required from a specialist, the doctor will discuss referral with you. Any associated costs are outside the normal framework of the medical and will be invoiced to yourself separately.

What to expect:

The medical appointment usually lasts for two hours. You will see the nurse or exercise physiologist for approximately half an hour, followed by the doctor for approximately an hour, and then have an Exercise ECG which lasts a further half an hour. The doctor will conclude the consultation, by summarising your assessment and providing you with practical advice to enhance your health and wellbeing.

During your appointment our doctors review your medical questionnaire and blood results. If you have any medical records or reports such as X-rays that you think are relevant you are welcome to bring this information with you to the appointment. Our doctors will discuss your health and wellbeing and may ask you for more information relevant to your condition and past history. A top to toe physical examination and the Exercise ECG are then carried out.

On conclusion of your appointment, the doctor writes a comprehensive report that includes information from your questionnaire and blood tests, as well as information from your interview and physical examination. The report discusses your health and wellbeing, and recommends any lifestyle changes, or treatments. This report is written in ‘lay-man’s’ terms, so that it is easy for you to understand. Medical terminology is avoided wherever possible.

Generally speaking, you will need to see your GP to arrange any recommended treatments that we suggest. However, we do provide medical follow-up and have a network of medical specialists that we are more than happy to refer you to if you don't have a regular GP.

Our report is sent to you, at your home address, unless otherwise specified by you during your appointment. A copy will also be sent to your GP, if you request it.
The turn around time for the report is two working weeks.

Personal or health information is not released to your employer or any others without your authority, except when that information is directly relevant to your health at work. For example, if it is deemed to be vital to ensure your safety and/ or maintain your health, or to maintain the health and/ or safety of other people. (The Health Information Privacy Code 1994).

Where a report is requested by your employer, you are entitled to discuss exactly what information will or will not be contained in the report with the doctor at the time of your assessment.

You are also entitled to a request a copy of our report and any information we have about you on file, at any time.

The responsibility for payment for this assessment and report is with the person or company who has requested the appointment. If you have been referred by your GP or you requested the assessment personally then payment of the invoice is your responsibility.

If you have any questions about this information or about what information will be included in our report please feel free to discuss this at the time of your appointment or phone us on 09 361 2396.
